HOW TO PASS THE BOARD EXAM? Easy, just read the guidelines and follow instructions..


IMPORTANT GUIDELINES in TAKING THE BOARD EXAM..

The following are the instructions or procedures to be followed by the examinees. It is understood that the Room Watchers will only assist in the writing of samples on th blackboard, distribution and collection of examination forms, test question sets, refer clarifications on the test question to the floor Supervisor and ensure that the procedures are followed by the examinees. However, you may also ask for clarification /assistance from the Room Watchers.
1. Upon arrival in your assigned room:
a. Place your bags (with your cellular phone inside), books, reviewers and other printed
materials OUTSIDE the room. The room watchers shall not be held liable for lost item.
b. From your Notice of Admission, detach the Applicant's stub, official receipt and mailing envelope.
c. Put your Notice of Admission, Official Receipt and other valuables inside your transparent envelope. All food and bottled mineral water, with their labels removed may be placed in front of your seat.

2. Distribution of examinee identification and answer sheet (ID/AS) set
a. Check if the serial numbers of the set are the same in all pages; if not ask for replacement
b. Check if the PRC seal and staple wire is intact front and back and the number of pages of the set from your Room Watcher.
3. Filling-up of the Identification (ID) Sheet (Insert your brown envelope in between the ID Sheet and the Answer Sheet to prevent indentation on the answer sheet.
a. Read the instruction printed on the ID Sheet. Do not detach this.
b. Start printing your names always on the first box of each column using BLACK BALL PEN and then mark the corresponding letters directly below each box using pencil. See sample of markings on the bard and ID sheet. If you have two names, separate the names by leaving one box vacant. Refer to the NAMEGRID sample printed on th blackboard.
c. In the box labeled YEAR & APPLICATION., copy the year and the application number from your applicant's stub, using BLACK BALL PEN. Print the last 2 digit of the year. Print 0 on the first box and 4 on the second box for year 2004 and print the 6-digit application no., from your application's stub.
d. Mark the corresponding numbers directly below each box using pencil.
e. See sample of markings on the board and ID Sheet.
f. Accomplish back portion of the ID sheet, using black ball pen. DO NOT DETACH YOUR ID SHEET. Fill-in all boxes, refer to separate instruction sheet. To protect your ID sheet, use your brown envelope as writing board.

4. Printing of name on your brown envelope and detaching of answer sheet (Before detaching the answer sheet, check if the brown envelope and ID/AS set is yours)
a. ON your brown envelope print your name in BOLD LETTERS in this order: LAST (FAMILY) NAME, FIRST NAME, MIDDLE INITIAL on the upper left hand corner and print your SEAT NO., on the upper right hand corner using BLACK BALL PEN.
b. Detach one (1) Answer sheet starting from the last page. Do the same in the next subjects. In detaching answer sheet, fold along the perforated line then detach. BEFORE DETACHING AN ANSWER SHEET, CHECK IF THE SET IS YOURS.
c. Return the remaining ID/AS set and your applicant's stub to your brown envelope. Extra papers, program of examination and examinees test taking guide must be in your brown envelope. The room watchers will collect your brown envelope at the start of the exam.

5. Filling up of the answer sheet
a. Fill up the data asked for in the box of your answer sheet by using black ball pencil capital letters, refer to the sample written on the right side of the board.
b. Read carefully the instructions on the face of the answer sheet "DO NOT WRITE YOUR NAME, SEAT NUMBER OR ANY IDENTIFYING MARKS OTHERWISE TOUR EXAMINATION WILL BE CANCELED . ONLY WRITE THE CORRECT SUBJECT TITLE.
c. In the box labeled SUBJECT, print the complete subject title in CAPITAL LETTERS using BLACK BALL PEN. Refer to the sample written on the right side of the board.
d. After filling-up your Answer Sheet, keep your answer sheet safe and clean inside your brown envelope whole waiting for the receipt of the test booklet and get it at the start of the exam. The room watchers will collect your brown envelope after the distribution of the test booklet.

6. Distribution and receipt of test booklet (You will receive your test booklet (TB) either set A or set B. After receiving your TB, do the following.
a. Count and check the number of pages of the set according to the page number. Check for missing pages, misprints, etc. Be sure it is complete, if not refer to your room watcher for replacement.
b. Print your SEAT NO. on the upper right corner of the first page of your TB, using pencil.
c. Mark the SET BOX on your Answer Sheet with the set of the Test Booklet you receive. If you receive Set A test booklet, mark Box A on your answer sheet, using pencil; if you receive Set B mark Box B on your answer sheet, using pencil.

7. During the process of examinations - talking, conversing with each other, using cellphone, making signs or other unnecessary movements are strictly prohibited.
a. Place your answer sheet ALWAYS ON TOP OF THE ARMCHAIR. Your pencil, black ball pen and non-programmable calculator may be placed on the floor in front of your seat.
b. Read the instructions printed on the Test Booklet.
c. You may answer directly on your answer sheet or use the test booklet sheets as your scratch using pencil. but be sure to transfer your answers to the answer sheet within the prescribed schedule/time. You can underline the important words or phrases on your TB, using pencil.
d. Follow the sample on the blackboard on the answer sheet for proper marking.
e. Strictly no erasures so as not to invalidate your answers.
f. Use pencil in marking the letter of each of your answers.
g. Be sure the page/s of your test booklet do not dangle from your armrest or desk.
h. Be conscious of the time. Look ate the schedule and time check written on the blackboard. Final time check will be announced one hour before the end of the exam and to be announced again thirty minutes before the end of the exam.

8. Submission of the test booklet and answer sheet- Be sure that you marked the Set Box (A or B) on the answer sheet, and fall in line to submit your notice of admission, answer sheet and test booklet intact to your Room Watcher.
a. Sign on the EXAMINEES' RECORD OF ATTENDANCE using your own ball pen. Leave your pencil on your desk. Indicate the actual time submitted and the number of pages of the Test Booklet.
b. Room Watcher 1 will sign and return your Notice of Admission. Bring your Notice of Admission in every subject.

NOTE: FAILURE TO SUBMIT YOUR TEST BOOKLET SET WILL BE A GROUND FOR DISCIPLINARY ACTION/CANCELLATION OF YOUR EXAMINATION.

9. Leave the room quietly, immediately after submission. Come back thirty minutes before the start of the next subject. Do not loiter or wait for your companion in the hallway. DO NOT OPEN YOUR CELLPHONE UNTIL YOU ARE OUT OF THE BUILDING OR TESTING CENTER.

10. Before the start of the next subject, do the following:
a. Get your brown envelope from your Room Watcher.
b. Check the name printed on the envelope and on the ID Sheet. Be sure it is your envelope with your ID sheet.
c. Follow procedures Nos. 4.b-c, and No.5-9
